Caso Clínico
• DCM, sexo masculino, 61 anos, branco, natural e procedente de
Salvador-BA, técnico de computação.
• HMA: Paciente portador de DCPF secundário a Vírus C da
Hepatite genótipo 1 + CHC (01 nódulo de 2,0cm em segmento
VII), com diagnóstico há 06 anos; inclusão em lista de transplante
há 01 ano.
• Realizou transplante hepático em 29/11/06.
Exames laboratoriais (pré-transplante)
• Anti-HCV: positivo
• AgHBs , FAN, anti-músculo liso, Anti-LKM1, HIV, HTLV: negativos
• CMV IgG: positivo
• CMV IgM: negativo

recorrência da hepatite C.

• Introduzido IFN-δ + Ribavirina
Acompanhamento Ambulatorial
• Março (2007): PCR HCV quantitativo : > 850.000UI/mL
• Genotipagem HCV: tipo 1
• Julho (2007): PCR HCV quantitativo 12 ª semana - > 850.000UI/mL
• Paciente considerado não respondedor ; optou-se por interromper o
tratamento anti-viral (fez uso deste tratamento por 27 semanas)
• Abril (2008): Carga viral: > 16.000.000UI/mL
• Julho (2008): Biópsia hepática: ausência de rejeição celular aguda,
recorrência da hepatite C crônica (Metavir A1 F1)
• Re-introduzido IFN em 26/06/09

ambulatorial
• Carga viral (01/02/10): 76.000.000 UI/mL
• Genótipo VHC: tipo 1 (01/02/10)
