RESUMO
A Auditoria de Prontuário em enfermagem, no setor de grande complexidade como
a UTI, envolve uma investigação de análise e avaliação dos registros de enfermagem não
apenas voltado para o conhecimento dos custos hospitalares, mas, sobretudo para a aquisição
de informações sobre assistência prestada ao paciente, contribuindo na promoção da
qualidade da assistência e facilitando o trabalho da equipe de enfermagem. O estudo realizado
foi de uma pesquisa de campo por base documental, que teve como principal objetivo analisar
os registros de enfermagem através de Auditoria em Prontuário na UTI geral de um hospital
privado do município de João Pessoa/PB. Como instrumento de coleta de dados utilizou-se
um roteiro para verificar os dados de identificação do paciente, informações sobre os
procedimentos realizados, aprazamento e checagem da prescrição médica, fechamento diário
do balanço hidríco, relatório de enfermagem, anotação dos tipos de altas e óbitos, organização
e identificação dos profissionais de enfermagem nos prontuários. A amostra foi compreendida
de trinta prontuários de pacientes com saídas (altas e óbitos) de uma população de 100%
destes, realizada nos meses de junho e julho de 2010. Os resultados demonstraram que os
registros de identificação do paciente estavam 100% preenchidos corretamente. Os registros
dos procedimentos de enfermagem como histórico de enfermagem com 73,33%, encontravamse corretos; as prescrições de enfermagem com 2,16% corretos; as evoluções de enfermagem
com 83,54% corretos; o aprazamento e a checagem da prescrição médica com 100%
corretos; balanço hídrico com 89,61% corretos; anotações de tipos de alta com 100%
corretas. Na organização e identificação dos profissionais, 100% dos registros abordavam
legibilidade, 3,04% rasuras, 30,28% com objetividade dos relatos, 98,51% com assinatura
do profissional e 95,96% com carimbo. O resultado da pesquisa mostra que o preenchimento
dos registros de enfermagem foi satisfatório no âmbito geral. Porém, o resultado obtido no
quesito prescrição de enfermagem constitui uma situação-problema para os gestores e
auditores da instituição. A partir dos problemas identificados, conclui-se que há necessidade
de maior investimento da auditoria de enfermagem na conscientização dos profissionais de
enfermagem através de sua função educadora e não apenas fiscalizadora e punitiva, e ainda,
da intensificação da educação continuada sobre registros de enfermagem, de forma a garantir
a qualidade da assistência prestada e a diminuição de custos hospitalares.

ABSTRACT
The nursing notes in patient promptuary in sectors, like the ICU, involves an
investigation of analysis and evaluation of the registers of nursing, not only towards the knowledge

of the hospitable costs, but especially to the acquisition of information about the assistance
given to the patient, contributing to the promotion of the quality of that assistance and thus
facilitate the work of the staff of nursing. This is a field research whose main aim was to
analyze the registers of nursing through the Promptuary records in the general TCU of a
private hospital in João Pessoa/PB. As an instrument for data collection, a route was used to
verify the data of identification of the patient, information about the procedures, stated hours
for medicaments and examination of the medical prescription, daily closure of the hydric
balance, nursing written report, notation of the kind of discharge from hospital and deaths,
organization and identification of the professionals of nursing in the promptuaries. The sample
consisted of thirty promptuaries of patients with depart (discharges and deaths) from a
population of 100%, accomplished between june and july, 2010. The results showed that the
registers of identification of the patient were 100%, filled correctly. The registers of the
procedures of nursing as description of nursing with 73,33% were right, prescriptions of
nursing with 2,16% right, evolutions of nursing with 83,54% right, stated period and examination
of medical prescription with 100% right, hydric balance with 89,61% right, notations of kind
of discharge from hospital with 100% right; in the organization and identification of the
professionals, 100% of the registers broached readable ones, 3,04 erasures, 30,28% with
objectiveness of reports, 98,51% with the signature of the professional and 95,96% with the
due seal. The result reveals that the filling of the registers of nursing was satisfactory at the
most, however the obtained result in the question prescription of nursing shows a problematic
situation for directors and auditors of the institution. From the identified problems, it was
concluded that there is a great necessity of a bigger investment in nursing notes concerning the
conscientiousness of the professionals in nursing through their educative function; not only
from their investigative and punishing function but still, from the intensification of a continued
education about the registers of nursing to guarantee, therefore, the quality of the given assistance
and decrease of hospitable costs.
